Board-and-care homes vs. assisted living communities

One of the most confusing things about senior care in California is that “assisted living” covers two very different kinds of place — and both hold the same state license (an RCFE, Residential Care Facility for the Elderly). Knowing the difference is the first step to choosing well.

Board-and-care homes

A board-and-care (sometimes called a residential care home or “six-bed”) is a private house licensed to care for a small number of residents — often six or fewer. You get a home-like setting, a high caregiver-to-resident ratio, and home-cooked meals.

  • Best for: someone who needs a lot of hands-on help, is frail, or does better in a quiet, intimate setting.
  • Trade-offs: fewer organized activities, less on-site amenity, smaller staff (so continuity depends on a few people).
  • Cost: often more affordable than a large community for the same level of care, though pricing varies widely.

Assisted living communities

A large community is an apartment-style building with dozens (sometimes hundreds) of residents, a full activity calendar, dining venues, transportation, and often a dedicated memory-care wing.

  • Best for: someone fairly independent and social who wants amenities, programming, and an active peer group.
  • Trade-offs: a lower caregiver-to-resident ratio, and care is often priced in tiers on top of base rent.
  • Cost: higher base cost, with add-on care levels; ask exactly what the base includes.

How to choose

  • Match the setting to the person. Social and independent → community. Frail or needs close attention → board-and-care.
  • Compare total cost, not base cost. Ask both types what care levels cost on top of the base rate.
  • Both are licensed the same way — so check each one’s license status and inspection record regardless of size.
  • Visit both kinds. Many families are surprised which one feels right once they walk in.
