What PACE is

The Program of All-Inclusive Care for the Elderly (PACE) is a Medi-Cal/Medicare program providing comprehensive medical and social services, allowing eligible seniors who’d otherwise need nursing-home-level care to keep living at home. It only operates in specific service-area zip codes, through specific licensed PACE organizations — not countywide or statewide.

Organizations serving parts of Sacramento County

Per DHCS’s own “California PACE Plans” service-area document (dated February 2026), three organizations currently list Sacramento County zip codes as part of their service area:

  • InnovAge Greater California PACE — Sacramento
    (844) 945-0467, 3870 Rosin Ct, Sacramento, CA 95834. A large zip footprint across central and east Sacramento County.
  • Habitat Health (Tungsten Health Sacramento, LLC)
  • Sutter SeniorCare PACE — the long-established, historically Sacramento-region-based PACE provider, still listed as active per the February 2026 DHCS document.
